Patient Safety Prize Finalists Announced

Elevance Health Foundation has selected five organizations advancing to the final round of its inaugural $5 million Patient Safety Prize.
The Elevance Health Foundation has announced the five Finalists for its inaugural Patient Safety Prize. Chosen from organizations across the country, these finalists are enhancing community-informed solutions to improve patient safety by reducing medication errors, strengthening health literacy, and preventing falls. Up to three Winners will each receive $1.5 million, with up to two Honorable Mentions receiving $250,000 to help expand their impact.
- Care Synergy – CAPABLE National Center designed CAPABLE (Community Aging in Place – Advancing Better Living for Elders) to prioritize client-directed outcomes of an older adult to remain safe in their home.
- NYU Grossman School of Medicine developed SAFER-Med, an AI-powered, multilingual teach-back system that helps patients better understand and safely manage medications after leaving the hospital.
- Parkland Center for Clinical Innovation created PARADE 360, which uses AI and community-wide data to identify patients at risk for adverse drug events before harm occurs.
- The Population Health Innovation Lab, a program of the Public Health Institute, is improving cardiac health literacy through community health workers serving people experiencing homelessness and substance use disorders.
- University of Pittsburgh developed SPARTA, which uses spatial AI during primary care visits to identify fall risk and connect older adults with preventive care.
Why did the Elevance Health Foundation create the Patient Safety Prize?
Patient safety remains one of healthcare's most significant challenges, with preventable harm affecting millions of people every year. Through its Community Action Leadership initiative, the Elevance Health Foundation launched the Patient Safety Prize to identify and accelerate proven, community-informed solutions that can improve care for people across the country. The inaugural competition focuses on three priority areas:
- Empowering Health Literacy for Safer Patient Care
- Innovating to Eliminate Medication Errors
- Promoting Fall-Free Futures
What happens next?
The Finalists will refine their proposals, submit a final pitch to the Selection Committee, and compete for up to $5 million in awards. The Foundation expects to recognize up to three Winners with $1.5 million each and up to two Honorable Mentions with $250,000 each later this fall.
