Libby Hoy
Libby Hoy is a nationally recognized leader in patient family engagement practice, leading PFCCpartners to embed sustainable PFE programs that promote equity and inclusion across health system activities, including research. Libby has extensive experience leading PCORI Eugene Washington Engagement Awards, including the Improving ICU Communications (2024), Patient Family Advsiory Summit on Measurement (2020), and the Patient Priorities Care (2016) projects. Libby leads the development, evaluation and sustainability of patient family engagement programs for the AGING Initiative (National Institute of Aging) and the Improving ICU Communications (National Institute of Health) projects. In these and all efforts to build patient family engagement practices, Libby is dedicated to building continuous learning loops, advancing health equity through inclusion and embedding the infrastructure to support productive and sustainable partnerships with patients, family caregivers and communities.
